Using Data-Informed Approaches to Address SDOH

Social Determinants of Health (SDOH) are the conditions and environments in which people live, work, and play that have a significant impact on their health outcomes. These determinants include factors such as access to quality education, safe and affordable housing, nutritious food, and reliable transportation. It is important for healthcare organizations to focus on social determinants of health because of their impact on health outcomes and health disparities across different populations. By addressing social determinants of health, healthcare organizations can move beyond episodic care and help to improve the overall health and well-being of their patients and communities.

Moving toward a value-based model whereby health organizations aim to impact SDOH is no small task for existing systems that are already overwhelmed by the state of chronic diseases and pandemic related impacts. It often takes collaboration and different vantage points to identify key opportunities to move the dial toward a more equitable and accessible health system.

Engaging outside experts can accelerate this process by helping organizations leverage data in conjunction with an organization’s experience and expertise to implement quality improvement plans that address social determinants of health and health equity. Data-informed approaches empower organizations to identify areas for improvement and evaluate the impact of their actions so they can make meaningful decisions that benefit the communities they serve for generations to come.
